My office is centrally located, close to the Columbia Center Mall in the Tri-Cities area of Washington State. I am accepting new clients at this time, but I am currently wait listing them. For those that are interested in starting the therapy process with me, please fill out the forms below. I’m making these available online so you don’t need to spend 15+ minutes in my waiting room filling them out before your appointment. They are:
- My Disclosure Statement-only need to bring the last page
- My office policies
- Depression Screening
- Anxiety Screening
- Alcohol Screening
- Fill out this screening form instead of the anxiety form if you are concerned you suffer from the effects of trauma or abuse
Print out the forms you’ve filled out and signed and bring them to your first appointment, along with your insurance card and a list of any medications you are taking. If you’re having trouble printing out the forms don’t worry, I have copies at the office. My office address is 8121 W Quinault Ave, Suite F202 in Kennewick. I have mapped it below for you as well. Insurances accepted: Asuris, First Choice Health, Group Health/Kaiser Permanente, Lifewise of Washington, Lifewise of Oregon, Medicare (for those 65+), Premera/Regence Blue Cross and United Health Care. I also accept Cigna and TriCare as an out of network provider.
