It’s easy to get started with me, simply call my office directly at 509-531-6698 to start the process. My office hours are 9am – 5pm, Monday to Friday. You are also welcome to contact me via the form at the bottom of this page with questions.
For those interested in starting the therapy process with me, please print and fill out the forms below. Please arrive 15 minutes early for your first appointment so we can begin on time. If you have not completed your paperwork I may reschedule your appointment. The forms to sign or complete are:
- Anxiety Screening. Fill this or the trauma form out
- Trauma/abuse form instead of the anxiety form if you are concerned you suffer from the effects of trauma, abuse or PTSD.
- Release of Information. If you want anyone else to have contact with me.
Print out the forms you’ve filled out and signed and bring them to your first appointment, along with your insurance card and a list of medications you are taking. If the forms are not printed and filled out when you arrive for our appointment, your appointment may be rescheduled unless you have made prior arrangements.
I recommend you call your insurance company to see if you will be paying a copay, coinsurance, or out of pocket if you have a deductible that has not been met. Remember that payment is due the day of service. If you do not have insurance or prefer not to use it, my rate is $200 for the initial evaluation and $130 for a regular session.

Insurances accepted: First Choice Health, Kaiser Permanente (PPO only), Lifewise of Washington, Lifewise of Oregon, Medicare (for those 65+ and not on disability) and Premera/Regence Blue Cross.
I only accept Aetna, Cigna, Medicaid/Apple Health, TriCare and United Healthcare as an out of network provider. This means you will pay my full fee, and may be reimbursed for a portion of it by your insurance company later. Call your insurance to verify this.
